Our practice specializes in Gender Affirming Care. We believe that all patients deserve accessible, equitable and competent care. We work with our patients to create a personalized transition profile based on individual goals and current best practices. No two patients are identical and each person deserves to play an active role in achieving their transition goals.
Given the shortage of providers that specialize in Gender Affirming Care, we have elected to offer services for both members of the DPC practice as well as non-members.
In addition to the many other benefits outlined on the website, the Gender Affirming Care Plan also includes:
- Guaranteed follow-up visits every 3 months
- As-needed telemedicine visits if any issues/ questions arise
- Letters for name and/or gender marker changes
- Medical letter of support for surgery

The initial evaluation is a comprehensive visit that follows both WPATH and Pediatric Endocrine Society guidelines. The visit involves both the patient and the patient's family members. Our goal is to focus on healthy family dynamics to ensure a safe and supportive gender transition process for our youth. This evaluation includes one in-person office visit and one follow-up tele-health visit.

In order to ensure a safe and effective transition process, patients will follow-up every 3 months for a visit. During these visits we will:
- Discuss the progress of the patient’s transition and if they are meeting their goals
- Continue to explore healthy family dynamics and provide support to both patient and family
- Psychosocial risk assessment
- Discuss any possible adverse side effects and what can be done to help alleviate them
- Analyze follow-up labs to ensure a safe and effective transition

For patients who are brand new to Gender Affirming hormone therapy. The initial evaluation is a comprehensive visit that follows both WPATH and Endocrine Society guidelines. We are providing two membership options for patient convenience.
