Pediatric/Adolescent (<18 years old)

Initial Evaluation

The initial evaluation is a comprehensive visit that follows both WPATH and Pediatric Endocrine Society guidelines. The visit involves both the patient and the patient's family members. Our goal is to focus on healthy family dynamics to ensure a safe and supportive transition process for our youth. This evaluation includes one in-person office visit and one follow-up telehealth visit. The evaluation includes: 


  • Review of patient’s past medical, surgical and social history
  • Review of patient's developmental history 
  • Review of patient’s current medications and allergies
  • Review of pertinent family history 
  • Organ inventory 
  • Exploration of gender identity development and gender diversity
  • Comprehensive biopsychosocial assessment 
  • Assessment and evaluation for the diagnosis of gender dysphoria (per DSM-IV criteria)
  • Comprehensive education regarding medical options for gender affirming care (puberty suppression, hormones, ect)  
  • Reproductive health and fertility options
  • Health risk assessment/stratification (once labs are completed) 
  • Creation of a personalized transition profile based on patient’s personal goals and current evidence based medical practice
  • Informed consent process for both patient and parents 
  • Injection teaching (if indicated)
  • Initiation of gender affirming medical care
